Hospital discharges, People with Dementia, People with Mental Health problems, People with Learning disabilities, People with Physical disabilities, People with Sensory impairment (Hearing; Visual), People with Addiction problems, People with Aids/HIV, People with MRSA, People who receive Palliative Care, Children and Young people with Special Needs, People who provide informal care to the forgoing, Families in need of support, People with Head injuries

We also deliver a service to numerous private service users throughout the country and can now provide direct nursing care services for and on behalf of the NHS and NHS Trusts.
